Our office is available to schedule appointments during the hours listed. We will do our best to schedule an appointment that is convenient for you:

Patient Forms
Prior to your initial visit, we ask that you complete the following forms:

• REGISTRATION FORMS
• USE AND DISCLOSURE OF HEALTH INFORMATION

Financial Responsibility
You are financially responsible for all services provided by our practice. These costs can include:

• Co-payments/co-insurance
• Deductibles
• Services not covered by your insurance plan
• Balance due on accounts

Payments can be made at the office or via mail. We accept cash, personal checks, Visa, and MasterCard.

Referrals
Please allow 48 hours for all referrals.

Prescription Refills
Please remind us at your next office visit if you need a refill on prescriptions. If you need a refill prior to that time, call your pharmacy and ask the pharmacy to fax us a prescription refill request form to: 773.735.5253
